Job Description:
HCA Healthcare is seeking a dedicated Physical Therapist who
specializes in Home Health to join our team. In this role, you will
be a critical part of our mission to deliver top-notch care in the
comfort of our patients' homes. With a focus on mobility
improvement, pain reduction, and prevention or management of
chronic conditions, you will provide personalized therapy services
tailored to individual patient needs. Our ideal candidate would not
only uphold HCAs high standards of care but also actively
contribute to a compassionate and supportive environment for
patients recovering at home. Assess patient conditions by examining
their medical history and any recent surgeries or procedures.
Develop customized treatment plans based on patient assessments and
goals. Implement physical therapy interventions, adjusting as
needed based on patient response. Educate patients and their
families about treatment plans and ongoing management strategies.
Maintain patient records and document patient progress. Collaborate
with other healthcare professionals to ensure comprehensive care.
Comply with all state regulatory requirements and HCA Healthcare’s
policies. Expertise in musculoskeletal, neurological, and
cardiopulmonary therapies Strong interpersonal and communication
skills Ability to handle physical demands of the job including
movement, transporting and guiding patients Proficiency in using
electronic health record systems Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Valid Physical Therapy license
in the practicing state CPR certification recommended
