With over 60,000 associates, there's a place for you to make a difference in lives of the families we serve throughout our continuum of care.ResponsibilitiesThe Case Manager position provides comprehensive clinical and rehabilitation updates with payer groups including managed care and BPCI/Bundled payment plans. Case managers ensure appropriate levels of reimbursement for patient specific care needs. This position assists with care transition of patients to the next level of care.Provides excellent written and verbal communication to patients/families, inpatient case managers, providers, and other nursing team members. Manages the case management processes from admission through patients' next lower level of care and understands payer contracts. Negotiates rates for improved reimbursement if necessary to capture additional care needs and ancillary services to support patients' care needs in a timely manner. Partners with nursing to collaborate with health care specialists to identify necessary ancillary services and support. Collaborates with the interdisciplinary team to provide efficient and high quality transition of care tasks through the continuum. Attends PPS and Weekly Medicare/IDT meeting to ensure patient care needs and payer expectations are coordinated. Provides assistance to patients for discharge planning, including but not limited to home health care, community health and/or community resources. Assists with providing patient specific education using the "teach back method" to ensure there is a full understanding of the discharge summary by the patient and/or family. Assists with medication reconciliation/education upon discharge using the "teach back" method to ensure there is a full understanding of the medication changes/new regiment by the patient and/or family. Provides the primary care physician (PCP) with a discharge packet and update on services/care the patient received in the community. Ensures PCP appointment is scheduled within 7 days of discharge from the community. Assists with setting up transportation to PCP follow up appointments.Provides post discharge follow up with patient/families and evaluates understanding of the discharge plan of care. Demonstrates compliance with facility and departmental safety and security policies and practices. Assists with educating hospital team members and physicians on case manager role at the community.This job description represents an overview of the responsibilities for the above referenced position. An associate should perform all duties as assigned by his/ her supervisor.QualificationsEducation and ExperienceA nursing license is preferred for this role; however, experience in social services, medical office billing and care transition experience will be considered. Ability to multi-task in an organized manger with a high degree of flexibility required. Ability to exercise critical thinking and make decisions in a fast paced environment is a must.Knowledge and SkillsPossesses extensive knowledge of a distinct skill or function and a thorough understanding of the organization and work environment. Has working knowledge of functional discipline. Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment. Ability to read and interpret documents. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the company.
